The main purpose of the assessment is to provide you with the best care possible by identifying your healthcare needs and sharing them with your doctor.
I see my doctor already. Do I need this?
Yes. This visit complements the regular care that you already receive. Your doctor will receive a summary of your visit with the nurse practitioner. This assessment acts as a checkpoint for your doctor, giving them insight into your current health and a head start on your next visit.
What happens during the visit? What does it involve?
This visit will be conducted by a nurse practitioner and usually takes about 45 minutes. The provider will review your health history and medications to build a comprehensive clinical summary of your health. This assessment will then be shared with you and your doctor to review. The provider will also review care programs and/or services available to you to help you better manage your health.

How does a telehealth visit work?
This is a virtual visit. Face-to-face interaction is done through a video call, which is accessed through a simple link (no app download is required). Seeing and interacting with you is a vital component of an annual wellness visit. This allows the provider to complete a physical assessment as part of this appointment. You will be able to see the provider, as well as ask any health-related questions that you may have.
What does this cost, and will it affect my coverage?
There is no cost for this assessment, which is covered as a preventive service under your insurance and aimed at keeping you healthy and well.
